Position Description

In this role, you will be assigned to a facility or facilities in your local community, allowing you to build meaningful connections with the individuals you serve. You will have the opportunity to conduct in-person psychiatric assessments, medication management, and implementing Medicare compliant gradual dose reduction strategies for psychotropic medications. Key Responsibilities:

  • Conduct comprehensive psychiatric evaluations and mental health assessments for patients in Long Term Care facilities, ensuring continuity of care.
  • Develop and implement individualized treatment plans, including prescribing and managing psychotropic medications.
  • Collaborate with interdisciplinary healthcare teams to coordinate care and optimize outcomes.
  • Monitor and manage chronic mental health conditions such as depression, anxiety, bipolar disorder, and schizophrenia.
  • Maintain accurate documentation of patient visits, treatment plans, and progress notes using electronic medical records (EMR).

Qualifications:

  • Current licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP or PMHNP-BC).
  • A Valid DEA Number
